Transaxillary Robotic Total Thyroidectomy for Thyroid Cancer

SUMMARY: A 39 years old female presented a 2 cm nodule in the right thyroid lobe. A fine needle aspiration cytology revealed the highest risk for cancer (TIR 4).
The procedure starts with a 5 cm longitudinal incision along the anterior pillar of the right axilla.
A subcutaneous tunnel above the pectoralis major muscle fascia is performed to reach the right lateral cervical region.

SUMMARY: The pneumoperitoneum is induced with the Verres needle and the abdominal exploration does not show carcinomatosis and liver metastases.
The patient is placed in a modified lithotomy position and then tilted in a steep Trendelenburg position.
A medial-to-lateral mobilization of the left colon represents the first part of the operation followed by high ligation with Hem-o-lock and section of the inferior mesenteric vessels.
