Once the esophagogastric junction has been clearly visualized the myotomy is started onto the proximal esophagus using the cautery hook. The myotomy is extended above the esophagogastric junction for about 6 cm and below for about 2- 2,5 cm onto the gastric side. The anterior branch of the vagus nerve is identified and preserved.</p>
